Black would be lost if it weren't for the perpetual check at his disposal:  1...Qe1+ 2.Kxf4 Qh4+

3.Kf5  White can repeat the position right away if he wants to: 3.Ke3 Qe1+, while 3.Ke5 transposes to the main line by 3...Qh5+ 4.Ke6 Qe8+.  3...Qh5+ 4.Ke6 Qe8+ 5.Kf5 Qh5+ and we have repeated the position after Black's third move. White cannot escape the perpetual check, and so offers Black the draw.
